The Leadership Team

David Chong - CEO

Coast Care Partners recently celebrated a decade of service in home care with David being at the helm since its inception. Leading up to this he has worn a few different hats in the field of geriatric care, having owned and operated The Park at Mount Helix as well as an Internet-based senior housing referral service, AssistMatch.

As a foundation to his career, David’s most useful degrees are his B.S. in Business Administration from Biola University and his J.D. from Boalt Hall School of Law at U.C. Berkeley. He also holds an M.A. in Practical Theology from the University of San Diego.  At various times he has held licenses in California as an attorney, a real estate broker,  an RCFE Administrator, and as a  Geriatric Care Manager with NAPGCM.

Board and other memberships have included the La Jolla Golden Triangle Rotary, the San Diego Regional Home Care Council, OASIS San Diego, Vistage, ProVisors and the Estate Planning Council of San Diego.

Some particularly favorite activities include scuba, backpacking, and hiking (focused on a hiatus from breaking bones). When not enjoying time with his Coast Care Partners partners, he can typically be found seizing the day with his wife Sarah and his four children.

Tyler Bowker - Director of Operations

Tyler is a Certified Administrative Assistant and started out as a Scheduling Coordinator with Coast Care Partners in August of 2016. She has since then grown her responsibilities into many roles in the office. Her current leadership role is to oversee the Scheduling Coordinators, Caregivers and other Human Resource tasks. She is currently working towards her Bachelor’s degree in Healthcare Administration. Prior to joining Coast Care Partners, Tyler worked as a Certified Nursing Assistant in Nebraska.

In her spare time, Tyler enjoys spending time with her four children: Austin, Brooklynn, Lucas and Parker, as well as their two dogs: Octavia and Mazikeen. She enjoys hiking, kayaking, and exploring new places.

Chris Jackson, RN - Director of Care Management

“Committed to excellence in your care experience while supporting quality of life in each and every day.”

Chris has been a Registered Nurse for 32 years.  She has a Bachelor’s Degree and a Public Health Nurse Certificate from University of New York Regents College.  Chris has devoted her clinical career of nearly 30 years to clinical care and management in the Home Care field as a Home Health Director, and Director of Patient Care Services in a large Hospice and Home Palliative Care organization. Her early experience also includes hospital acute care nursing with cardiac and pulmonary patients, as well as work in home infusion therapy, skilled nursing facility care delivery and disease management programs.

Chris found her passion for serving patients and clients in their home environment early

in her nursing practice, recognizing the value in their health, comfort and well-being in the familiar surroundings of home.

In her free time, Chris enjoys hiking, yoga, knitting, and spending precious time with family including her two young grandchildren.

Frank Velazquez - Director of Finance

Frank is a Southern California native with over 20 years of leadership experience in many cities across a variety of industries in  Accounting, Finance, Operations and Sales. He graduated from Cal State Fullerton in Accounting and earned his CPA while working at Ernst & Young.   He believes in a data-driven approach to achieving goals.  He is motivated by inspiring people he works with and seeing them realize their goals.

Frank believes in being an integral part of the community, participating in Chamber of Commerce and Rotary efforts and supporting many organizations who help those in need, including the Chino Neighborhood House.

On a personal note, he and his wife cared for his disabled mother-in-law who lived with them for nearly 30 years. In his spare time, Frank enjoys being outdoors with activities like camping, taking walks and traveling with his wife.

Care Management

Amy Klein Duane, LCSW – Care Manager

Amy has been a practicing Social Worker for over 30 years. She earned a BA in psychology from Syracuse University and her MSW from The Wurzweiler School of Social Work/Yeshiva University in New York City. She found her passion working with seniors at her first Social Work internship, providing services for homebound elderly in Manhattan. Amy has worked with chronically mentally ill patients in a day treatment facility, she has been a bereavement counselor at a local hospice and she has worked with children with emotional and behavioral challenges. She has also been a psychotherapist in private practice.

Amy enjoys hiking, reading, baking and spending time with her husband and four adult sons.

Pari Ghalebi, RN – Care Manager

Pari Ghalebi has over 20 years of experience in geriatric nursing. As a Registered Nurse, Pari chose the path of dedicating her professional expertise to the lives of seniors. For more than 15 years, she served as a Director of Nursing for three major senior health care facilities in Nashville, Tennessee, and more recently at White Sands of La Jolla. For the past five years, Pari has been applying all her clinical and social care experiences to Home Care Management. She has offered high standards of Care Management to many elderly who have chosen the comfort of their own surroundings with reliable care plans along with a team of kind and responsible caregivers.

Karin Morris – Associate Care Manager

Karin is the mother of four children of whom she is very proud. Karin was born in Germany but has been a California girl since she was three years old.

Karin was a licensed Massage Therapist specializing in injury recovery and prenatal therapies for 12 years. Before moving into management, Karin worked as a Caregiver for 10 years and worked as a Staffing Manager before joining Coast Care Partners.

In her spare time, Karin loves the beach, camping with her family and painting watercolors. She and her girls also raise puppies for Guide Dogs for the Blind Foundation.

Nubia Moreno – Associate Care Manager

Nubia has been working with the elderly for over 20 years, she started off as a caregiver working in facilities and also private care. She was presented with an opportunity for an Administrative role and took on the challenge.  She has since worked for several In-Home Care Agencies across San Diego including Senior Helpers, HomeWatch Caregivers, Comfort Keepers, Visiting Angels and now she has taken on the role of Associate Care Manager with Coast Care Partners and is hoping to build new relationships with clients and Home Care Aides.

In her free time Nubia spends time with her two sons Jayyden and Jayvyn and her fiance Brian. They like to try new restaurants, go to the movies and enjoy beautiful San Diego!

Sales Management

Holli Bennett – Client Services Manager

Holli is a native of San Diego and has over 24 years of experience in the senior care industry. She has worked in skilled nursing facilities, residential care facilities for the elderly and she has also marketed for a major ambulance company, but found her true passion working in-home care with seniors.

Holli held her Residential Care Facility for the Elderly Administrators License with the Department of Social Services and helped run a successful six-bed board and care.

Holli enjoys spending time off-roading and camping in the desert with her husband along with playing with her 2 grandchildren.

Michelle Guire – Client Services Manager

Michelle earned a Bachelor of Science with a double major in Health Fitness Promotion and Programming in Corporate and Clinical Settings and Psychology.  She excelled in pharmaceutical sales and sales training, with Pfizer and Eli Lilly.  Helping to provide patients options for their health care and wellness needs has long been a motivating factor for her.

She enjoys spending time with friends and family, traveling and taking yoga classes.

Claudia Centeno - Lead Scheduling Coordinator

Claudia had been working as a CNA for Coast Care Partners for three years before being promoted to Scheduling Coordinator. She has over seven years of experience in caregiving and loves to help the elderly. Besides helping with scheduling, she also does the employee reviews for the company’s Home Care Aides.

Claudia is a San Diego native, and in her free time enjoys going to the beach and spending time with her four kids and her grandson.

Andrea Salazar - Scheduling Coordinator

Andrea started with Coast Care Partners as a caregiver and quickly moved into the office as a scheduling coordinator. She originally became interested in senior care after discovering how rewarding it was to look after her  Dad and Aunt. Born in San Diego but raised in Tijuana, Mexico, Andrea is now pursuing a degree in Criminal Justice.

Odette Orduna - Recruitment Coordinator

Odette is a native of San Diego and holds a Bachelor’s in Gerontology from San Diego State University. She has over 14 years of experience working in the healthcare industry. Odette developed her passion as an intern bereavement counselor at a local hospice and since then has worked as a caseworker within local hospital systems and has amassed vast experience in coordinating, training, recruiting and developing teams.

She loves being involved in community events, if there is a walk or race somewhere in town, she has most likely volunteered to be a part of it.  She is deeply passionate about helping others within her community, which is why she is currently pursuing a Master’s in Public Health.

Robert O'Neal - Recruitment Coordinator

Robert is a native of San Diego and is pursuing a Bachelors in Abnormal Psychology. He began as a caregiver 3 years ago and developed an appreciation and passion for the administrative and hands-on work with individuals struggling with mental, physical, and developmental disorders. Outside of work he mentors individuals recovering from substance abuse disorder and is an active member in the recovery community.

In his free time he enjoys travelling, playing disc golf, and going to the beach.

Donna Scibilia - Direct Support Mentor

Donna has had over 10 years of experience in senior care. She was a full-time Home Care Aid with Coast Care Partners for 7 years, then was promoted to Care Coordinator. She now wears multiple hats including teaching, leading orientation, mentoring Home Care Aids, and visiting clients.

Donna is originally from the East Coast. She enjoys photography, dogs, dancing and hiking. She has 2 grown sons and a grand-dog, Bella.
